New Swimmer:

     A new swimmer must meet these qualifications:

                             Be at least 5 years of age
                             Must be able to swim 1 length of pool (25 yards)
                             Must be able to follow instructions
                             Did not swim with a USA Swimming club in the last year

                      Please complete these forms:

Ø    New swimmer form with  USA Swimming 2011 Athlete Registration Application

o       Please complete one form per swimmer ( please print )

o        LSC = Local Swim Club

o       Our  LSC =  Missouri Valley Swimming or MVS

o         Our Club Code =   SAC

Ø      Salina Aquatics Club Swimmer Information form

o        Please complete 1 form per family (please print)

o       Signed by parent/guardian

Ø      Medical Form

o        Signed by parent/guardian and notarized

Returning Athlete:

                 Swam in the previous year with Salina Aquatics Club

                    Please complete these forms: 

Ø      USA Swimming 2011 Athlete Registration Application

o        (For returning swimmers, USA Swimming 2011 Athlete Registration payment is due in December.  We accept payment anytime between September  – December )

o        For 2011, it is $52.

Ø         E-Z Renewal Form

o       If an athlete swam in the previous year with Salina Aquatics Club, this form may be used.

Transfer Athlete:

    Swam with a club team in the previous year

Please complete these forms:

Ø      Medical Form

o       Signed by parent /guardian and notarized

Ø      Salina Aquatics Club Swimmer Information Form

o       Please complete 1 form per family (please print)

o       Signed by parent/guardian

Ø      Transfer Release Form from Club 

Ø       USA Swimming 2011 Athlete Registration Application

o        if haven’t registered for the year

High School Swimmers _ come on and join; a discount of the SAC monthly fee may be offered for the weeks not swum in the month before high school boys/girls swim season starts.
